Santa Clara County Community Plan to End Homelessness 2020-2025
Why have we updated the plan?
Nearly five years ago, our community adopted the Community Plan to End Homelessness in Santa Clara County 2015-2020, which was developed collaboratively by representatives of community-based service organizations, local government, philanthropy, business, healthcare, and people with lived experience.
We are pleased to now present to you the 2020-2025 Community Plan. This is the product of a robust community engagement process led by the Santa Clara County Continuum of Care. For the next five years, this plan will guide the County, cities, nonprofits, and other community members as they make decisions about funding, programs, priorities, and needs.
